A demo sheet (face sheet) name, ss#, ins., dob, etc.
Copy of patient's insurance cards
Superbill, charge sheet, (includes narrative of procedure and dx and appropriate e&m code). We can provide a charge sheet if you need one.
Anesthesia Record copy.
